Admission
The YES Home is licensed to accept 10 youth into our program. YES is an emergency (ages 12-20) and long-term/residential (ages 14-20) group home. We are not licensed to accept youth who are sexually maladaptive.
Once accepted into the YES Home program, please complete the proper intake packet below. Fill in all boxes and enter your email address once completed. You will need to then verify your email address for Adobe, in order to complete the intake packet. If your email address is not verified, the packet will not be properly submitted.
